ATHLETIC ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY. IN CONSIDERATION OF my child/ward named above being allowed to participate in any way in the Al Amanecer Boxing Club related events and activities, the undersigned acknowledges, appreciates, and agrees that: 1) The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) to my child from the activities involved in these programs are significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and, 2) FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child’s participation; and, 3) I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child’s readiness for participation and/or in the program itself, I will remove my child from the participation and bring such attention of the nearest official immediately; and, 4) I myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Al Amanecer Boxing Club; its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property incident to my child’s involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. 5) I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law. 6) I, the parent/guardian, assert that I have explained to my child/ward: the risks of the activity, his/her responsibilities for adhering to Al Amanecer Boxing Club’s rules and regulations, and that my child/ward understands this agreement.
ATHLETIC ASSUMPTION OF RISK - Field(Required)
As the parent/legal guardian of a participant in Al Amanecer Boxing Club, I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry, if I am unavailable to be contacted to provide necessary consent at the time of incident. This care may be given under whatever conditions are necessary to preserve the life, limb, or wellbeing of the participant.

I hereby authorize Al Amanecer Boxing Club to publish photographs taken of my minor child/ward, and my minor child’s/ward’s name, and likeness, for use in Al Amanecer Boxing Club’s print, online and videobased marketing materials, as well as other publications. I hereby release and hold harmless Al Amanecer Boxing Club from any reasonable expectation of privacy or confidentiality for myself and/or any minor child and/or children listed above associated with the images specified above. I further acknowledge that participation is voluntary and that neither I and/or the minor child or children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in Al Amanecer Boxing Club’s marketing materials or other publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release Al Amanecer Boxing Club, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation and/or the minor children listed above.

I understand that I have 5 (Five) days from the date of entering this membership contract in which to cancel. Al Amanecer Boxing Club agrees to refund any fees paid if canceled within the aforementioned 5 days, less fair compensation for any services provided (up to a maximum of $100).

En consideración a que mi hija/o que se le permita participar de cualquier manera de Al Amanecer Boxing Club relacionados con eventos y actividades, el firmado reconoce, aprecia y acepta que: 1) El riesgo de lesions y enfermedades (ej: enfermedades transmisibles como MRSA,b COVID-19) a mi hija/o de las actividades involucradas en estos programas son significantes, incluyendo la posibilidad de discapacidad permanente, o muerte, y aunque normas particulares, equipaje, y disciplina podría reducir estos riesgos, los riesgos de discapacidad permanente enfermedad todavía puede existir; además, 2) PARA MÍ, ESPOSA/O, Y HIJA/O, YO ASUMO CONSCIENTE Y LIBREMENTE TODOS ESOS RIESGOS, tanto conocidos como desconocidos, INCLUSO SI SURGE DE LA NEGLIGENCIA DE LAS LIBERACIONES u otras, yo asumo toda la responsabilidad de la participación de mi hija/o; , 3) Estoy de acuerdo en cumplir con los términos y condiciones establecidos y habituales del programa para la participación. Si observo alguna preocupación inusual significativa en la preparación de mi hija/o para participar y/o en el programa mismo, retiraré a mi hija/o de la participación y informarles inmediatamente al funcionario más cercano; y, 4) Yo mismo, mi esposa/o, mi hija/o, y en nombre de mis herederos, cesionarios, representantes personales y familiares, POR LA PRESENTE LIBERO Y MANTENGO INOFENSIVO Al Amanecer Boxing Club; sus directores, oficiales, funcionarios, agentes, empleados, voluntarios, otros participantes, agencias patrocinadoras, patrocinadores, Anunciantes, y si corresponde, propietarios y arrendadores de locales utilizados para llevar a cabo el evento (“Reempleados”), CON RESPECTO A CUALQUIER LESIÓN, ENFERMEDAD, DISCAPACIDAD, MUERTE, o pérdida o daño a la persona o propiedad incidente de propiedad a la participación de mi hija/o o participación en estos programas, YA SEA QUE SURJA DE LA NEGLIGENCIA DE LAS LIBERACIONES O DE OTRA MANERA, en la máxima medida permitida por la ley. 5) Yo, por mí mismo, mi cónyuge, mi hija/o, Y en nombre de mis herederos, cesionarios, representantes personales y familiares, POR LA PRESENTE INDEMNIZAR Y EXIMIR DE TODA RESPONSABILIDAD a PRESENTE INDEMNIZAR Y EXIMIR DE TODA RESPONSABILIDAD a todas las personas mencionadas anteriormente de cualquier incidente de responsabilidad a mi participación o participación en estos programas, INCLUSO SI SURGE DE SU NEGLIGENCIA, en la mayor medida permitida por la ley. 6) Yo, el padre/tutor, afirmo que le he explicado a mi hija/o/pupilo: Los riesgos de la actividad, sus responsabilidades por cumplir con las reglas y regulaciones del Al Amanecer Boxing Club, y que mi hija/o/pupilo entiende este acuerdo.

Consentimiento para el tratamiento médico. Como padre/tutor legal de un participante en Al Amanecer Boxing Club, doy mi consentimiento para la atención médica de emergencia prescrita por un médico de medicina o médico de odontología debidamente licenciados. Si no estoy disponible para ser contactado para proporcionar el consentimiento necesario en el momento del incidente. Este cuidado puede ser dado bajo cualesquiera condición necesaria para preservar la vida, extremidad, o bienestar del participante.

Reconocimiento de liberación de fotos. Autorizo por la presente a Al Amanecer Boxing Club a publicar fotografías tomadas de mi hija/o menor y del nombre y semejanza de mi hija/o menor, para su uso en materiales de marketing impresos, en línea y en vídeo del Al Amanecer Boxing Club, así como en otras publicaciones. Por la presente libero y mantengo inofensivo Al Amanecer Boxing Club de cualquier expectativa razonable de privacidad o confidencialidad para mí y/o cualquier niño/a menor y/o niños/as enumerados anteriormente asociados con las imágenes especificadas anteriormente. Reconozco además que la participación es voluntaria y que ni yo ni el menor o los niños/as recibirán compensación financiera de ningún tipo relacionada con la toma o publicación de estas fotografías o la participación en los materiales de marketing u otras publicaciones del Al Amanecer Boxing Club. Reconozco y acepto que la publicación de dichas fotos no confiere ningún derecho de propiedad o regalías. Por la presente libero Al Amanecer Boxing Club, sus contratistas, sus empleados, y cualquier tercero implicado en la creación o publicación de materiales de la comercialización, de la responsabilidad por cualquier demanda por mí o cualquier tercero en relación con mi participación y/o de los niño/as menores nombrados arriba.

Entiendo que tengo 5 (cinco) días a partir de la fecha de entrada en este contrato de suscripción para cancelar. Al Amanecer Boxing Club acuerda reembolsar cualquier honorario pagado si cancelado dentro de los 5 días antes mencionados, compensación menos justa para cualquier servicio proporcionado (hasta un máximo de $100).
